Become a New Patient
Thank you for reaching out to inquire about becoming a new patient at Benjamin Liess MD, LLC. Our office requires all patients to have a referral from their primary care physician before we can set up an appointment. We kindly ask that you contact your primary care provider to request a referral to our office.
What Your Referral Needs
To provide you with the best possible care, your referral to our practice should include the following information:
- Name and date of birth
- Address/demographic information
- Name of the primary care physician and their practice information
- Current insurance information
- Start and end date of the referral
- Number of visits allowed
- Reason for visit/diagnosis
- A referral authorization number if the insurance plan requires
- Any office visit notes pertaining to the diagnosis
Next Steps
Once received, we will review your referral to make sure that our office is the right fit for your case. Please note, depending on the primary care office, the referral process from start to finish can take about two weeks in some cases, and we are currently booking out about 3-4 weeks.
